Office Information
 Before Your Visit
At OGI our goal is to make every patient visit a pleasant experience. To help us achieve this goal, please bring the following information with you for your appointment.
Insurance Card
Orthopedic Group participates in most major insurances. Be sure to bring your insurance card to each appointment. We will submit a claim to your insurance company for your office charges, injections, x-rays, and surgical procedures. Payment of non-covered office visits are due at the time of the visit and enjoy a 30% prompt payment credit when paid-in-full at time of service. Members of managed care plans that have a co-payment are required to pay that portion of their office visit charge at the time of their visit. We accept cash, personal checks, MasterCard and Visa. If your medical bills are to be covered under Worker’s Compensation, please call Genny in our Worker’s Compensation Department at (401) 722-2400 extension 3302 or Melissa at extension 3340.
Referral Form
If you are enrolled in an HMO, you may be required to obtain a referral/authorization form from your primary care physician before your office visit or surgery. To prevent you from being billed for services that are covered under your insurance plan, our office must receive the referral/authorization form before your visit. You can call your primary care physician and ask him to fax the form to our office at (401) 728-3920.
 Radiology Films
Orthopedic Group is proud to offer our patients state-of-the-art, digital x-ray services. Please be sure to bring all existing x-rays along with the written report (if available) that relate to the medical condition for which you are being treated. This may include CT scans, and MRI’S as well.

Patient Accounts
You will need to provide insurance information prior to your office visit. At the time of the visit, we will verify information, copy your insurance card, and discuss our policy for handling your specific type of insurance. Orthopedic Group will file insurance claims for all services covered under your plan. Non-covered services, co-payments or deductibles are the responsibility of the patient at the time of the visit. Also, for plans we do not participate in, you may be asked for payment of service at the time of visit. Orthopedic Group will arrange a budget payment plan if necessary. Our patient accounts staff is available in the office to personally answer billing and insurance questions, or can be reached by calling (401) 722-2400 extension 3313 (Marie), extension 3337 (Wendy), extension 3349 (Bette), extension 3357 (Thomas), extension 3356 (Kerri), or extension 3358 (Roxanne).
